The challenge
Tri-County's Section 330 grant came up for competition in a year with a new CFO and a grants manager eight months into the role.
Need and impact sections drew on UDS tables, EHR population reports and a 2023 community health needs assessment, none in the same format.
Previous applications had lost points on response to sub-criteria the team believed they had addressed elsewhere in the narrative.
What they did
- Uploaded UDS reports for three years, board minutes, the sliding fee discount program policy and the needs assessment.
- Mapped every SAC review criterion and Health Center Program requirement to vault evidence before drafting began.
- Built the budget narrative against the SF-424A so federal and non-federal shares reconciled.
